Under the shop construction/operation scheme, interested disabled people can apply for shop construction/operation online form at http//divyangjandukan.upsdc.gov.in. While filling out the online form, the applicant must submit the application form online on the website along with the latest photograph showing disability, income and caste certificate, age certificate (which has a date of birth mentioned), disability certificate issued by the competent authority, bank account operated in a nationalized bank, certificate of residence and photocopy of Aadhar card, etc. Also, get the print copy of the online submitted application form and hard copy of the desired forms to the office of the District Disabled Empowerment Officer.
